FAQ’s for Assisted Living

Active Adventures Assisted Living LLC is an Adult Family Home, licensed to care for up to six residents who need physical assistance due to a medical condition or age-related decline. Our staff have specialty designation and training in dementia (also known as “memory care”). Owned and operated by a Registered Nurse, we provide care all the the way through to end of life. Such care includes – but is not limited to – assistance with dressing, toileting, eating, medication management, and mobility, even up to maximum care in all areas.

 

Here at Active Adventures, our goal is stability and high quality of life for each of our residents. As an experienced RN, our owner, Heather, will supervise the care of your loved ones and ensure they receive close management and care planning for their individual physical and/or cognitive needs. Every week, we hold a care conference between Heather and the rest of our staff for each resident, to make sure every detail in your loved ones’ care is updated and catered to their ever-changing needs.

In the state of Washington, adult family homes are licensed to a maximum of 6 residents. Here at Active Adventures, we choose to have only 4 residents with 1 caregiver. Care is provided by a well-trained CNA 6 days a week and by Heather herself the other 1 day and all nights. Awake caregiving at night depends upon the assessed needs of the residents, but Heather is always present to attend to needs as they arise. Fall mats, siderails, bed alarms, and a call system are frequently in use to minimize the risk of falls and adverse events. Cooking and food preparation are planned in advance, with input from residents and family members, and a rotating perfected menu, so that ample time is available to attend to all of the care needs of your loved ones, as well as spend one-on-one time with them.

Many long term care facilities charge a monthly rate based on levels of care or a “points system.” However, here at Active Adventures, we charge an all-inclusive flat rate* based solely on initial assessment. This rate increases by 3% annually to keep pace with inflation, and only changes further if there is a significant change in your loved ones’ needs, that would require increased staffing.
We will be sure to provide transparent communication well in advance to you, the family, so there are no surprises.
If a resident moves in mid-month, Active Adventures will charge a prorated fee for that first month, so they may pay only for the days of the month that are reserved.
This monthly rate includes virtually everything, as we try to limit any extra add-ons. Each of our rooms is equipped with a flat-screen TV with streaming channels, meals and snacks, laundry, hair and nail care, activities, medication management. Nurse delegation and updates to assessments and care plans are completed by the provider at no extra charge.
*Please note: we do not accept Medicaid for new admissions. Only after a 2-year spend down will we accept Medicaid. Please contact Heather for details.
Here at Active Adventures, we feel that food is something to be enjoyed and celebrated. We have a rotating menu for lunches and dinners served at approximately 12-1 pm and 5-6 pm, depending upon our residents’ preferences. We also bake homemade breads and treats, with snacks and desserts available at all times. We encourage family recipes, food experiments, and fun in the kitchen as an activity to be enjoyed.
Breakfast is served per each resident’s schedule and preference. Residents are encouraged – but not required – to eat lunch and dinner together at the main table. This promotes conversation and a social experience during meals.

We aim to cook and serve the correct amount at each meal for our residents and caregivers, so we can minimize leftovers and waste. However, we can certainly accommodate 1-2 visitors joining with us for a meal on an occasional basis. We request at least a 48-hour notice, so that we can plan ahead to prepare more for that meal.

Every resident’s birthday is celebrated with a special table set up with a cake shared with other residents, flowers, and balloons. We also encourage family members to join us for these important celebrations, which expand to the large outdoor patio during warmer weather.
We welcome family members to our Mother’s Day Brunch, Father’s Day Lunch, Independence Day Meal, and the occasional summer BBQ. Residents are encouraged to voice their decoration and culinary inputs for these fun occasions.

Friends and family are welcome to visit at any time, though we request that visits be scheduled between 10 a.m. and 8 p.m.

We can use a long-term care pharmacy to deliver bubble-packed medications, similar to the larger facilities, or unique to this RN owned home, Heather, as an RN, can fill bubble packs herself with the medications needed and teach this to families. This allows residents to continue using their retail pharmacies, at a significant cost savings, and allows for direct communication and instant adjustments in medications as needed.

We keep abreast of the availability of visiting physicians as we learn of them. At this time, there are few services available, but Heather is a Nurse who can communicate on a professional level with all medical staff, to consolidate visits, to support telehealth calls and to assist Home Health services when needed.

Because Medicare rarely covers podiatry as often as needed, Active Adventures can provide a visiting podiatry-trained Registered Nurse who comes to the home every 8-10 weeks to provide basic podiatry care, which is included in the monthly rate. Between these visits, our caregivers can trim and file toenails and fingernails and provide manicures and polish as a weekly activity. Diabetic foot care is overseen by Heather.
We also recommend and can assist with establishing home visits by a dental hygienist at an additional cost as needed.
Active Adventures also works closely with a variety of Home Health and Hospice* companies around Clark County. Each company accepts new admissions based on physician orders, contracted insurance, availability, and resident/family preference.
*Hospice is generally initiated when your loved one has a change in status that is life limiting and there are no treatment options available or you, the family, choose not to continue treatment. It does not mean that care is fully withdrawn, or that death is imminent, as many individuals continue to live for months after hospice is initiated. Instead, hospice is a program which offers comfort and dignity at the end of life, providing the best quality of life at the end of one’s own. Active Adventures very much values the philosophy of quality care that hospice offers.

No. Active Adventures is a smoke, vape, and tobacco-free facility.

Yes, in moderation when our residents wish to enjoy a nightly glass of wine, beer, or a cocktail with/before dinner. However, we communicate with family and clinicians about the safety of alcohol use in regards to medications and disease processes. Caregivers are the only ones who pour and serve the alcohol if your loved ones choose to enjoy it.
We offer a gamut of activities here at Active Adventures. We have talented caregivers who can play music, and other musical activities throughout the week. After breakfast, caregivers assist residents with stretching and exercises. Group and individual activities are both scheduled and spontaneous, throughout the day and week. We continually explore new activities based upon resident likes, preferences and abilities.
Many residents of assisted living homes enjoy assisting with household operations, and our caregivers give your loved ones opportunities as such. Some of these opportunities include assistance in the kitchen, laundry folding, or other activities that are safe.
We also encourage family members to take their loved ones on any outings, if they feel the resident would benefit. At this time, we do not have scheduled group outings away from the home, due to safety and stress.

Included in our monthly rate is our visiting Beautician, who comes to our facility every 6 weeks to provide basic haircuts to each resident. Our caregivers will assist each resident with showers and hair washing twice a week, or more often per preference/need, and can assist as needed for styling each resident’s hair.

In the event of a medical emergency, the resident’s “code status” is taken into consideration, but it is the law that 911 is called for all medical emergencies in the home. As an RN with decades of experience, the nature of the emergency and the best course of action can be quickly assessed and acted upon. It is our goal to keep all family members informed of changes, and we aim to never send a resident to the Emergency Room alone unless that resident is alert and able to answer questions for themselves.
